INFORMATION FOR COACHES

For the check, please take this form to the police station within your jurisdiction.  You will be required to provide two pieces of identification, ONE WITH PHOTO I.D. (driver's license, photo health card) and ONE OTHER PIECE OF IDENTIFICATION (birth certificate, health card, etc.).  There may be a charge for the Police check which is payable to the police force.  At the Peterborough-Lakefield Police Department, we expect this to be charged to a “PYSC” list, but you might have to pay the record check and be reimbursed later.  If so, please get a receipt.  The Police Record Check must be shown to an executive member at one of the coaches' meetings.
