The PYSC registration process consists of the OSA Registration Rules, the PYSC Registration Rules, and the PYSC Registration Form. Please read the OSA Registration Rules and the PYSC Registration Rules before printing, filling out and submitting the PYSC Registration Form.

By signing and submitting both pages of the PYSC Registration Form the signer agrees to all rules as specified in the OSA Registration Rules and the PYSC Registration Rules.

After you have reviewed and agreed to the OSA Registration Rules (STEP 2) and the PYSC Registration Rules (STEP 3) you may print and submit the PYSC Registration Form below. Note that there are two pages to the PYSC form and both must be signed, dated, and submitted. The mailing address is on the form.
